前言
椎間盤脫出(disc herniation)是臨床常見病和多發(fā)病,其基本病理變化是椎間盤退變,在此基礎上受到外傷、受寒等外界因素刺激,導致椎間盤的髓核及部分纖維環(huán)向周圍組織脫出,壓迫相應脊髓或脊神經(jīng)根所致的一種病理狀態(tài)。目前椎間盤退變的病因及發(fā)病機制尚不清楚,也無有效的治療方法。動物模型是研究椎間盤脫出發(fā)生機制和治療策略的一種有效手段。用于建立椎間盤脫出動物模型的物種包括:小鼠、大鼠、家兔、犬、綿羊、豬、山 羊以及猴子等。構(gòu)建動物模型的方法通常有兩種:自發(fā)型和實驗誘導型。由于物種之間在細胞、組織、發(fā)育、生理、椎骨和椎間盤的解剖以及力學特性上存在很大差異,另外,不同的方法建立的動物模型與人類疾病表現(xiàn)也有所差別,因此,研究者在選擇動物模型時應盡量考慮多方面的因素對模型的影響。
部分造模方法
自體髓核移植法:將實驗動物自身的髓核取出,通過注射或手術(shù)等方法將其移植至脊神經(jīng)根和硬脊膜之間,從而模擬腰椎間盤脫出癥。自體髓核移植法已成為主流的腰突癥動物模型,廣泛應用于腰椎間盤脫出癥的發(fā)病機制以及某些神經(jīng)遞質(zhì)和炎癥因子甚至是一些藥物和治療手段作用機制的研究中。
基因修飾動物模型為椎間盤脫出提供了新的選擇。Ⅱ型膠原基因失活小鼠椎體終板不規(guī)則增厚,纖維環(huán)、終板和椎骨內(nèi)糖胺聚糖含量較正常小鼠少,并出現(xiàn)與人類相似的椎間盤脫出性改變。氯化筒箭毒堿(也稱為生長分化因子8)基因敲除小鼠肌骨骼生長的負調(diào)控,導致肌肉量與骨密度增加,蛋白多糖含量降低,椎體終板骨化,出現(xiàn)椎間盤脫出跡象。
